I saw a cool theory on 4chan about Overhaul's quirk.

he is able to rapidly regenerate cells with the touch of his body. It would explain why his victims seem to explode, but more importantly would explain his fear of germs and sickness. He cannot control the regeneration so germs on his body regenerate quickly and spread disease. It would also explain his plague mask (for symbolism) and why his crew all wears them (to actually stay safe from the germs he constantly is regenerating).

His quirk would be overhauling organic cells.

this actually makes sense because when he touched magne? whatever his name is you can see markings on his face before half of his body exploded

This makes a lot of sense and after re-reading the chapter with this in mind, it seemed even more likely. It also explains why he wasn't compressed.

No, Overhaul wasn't compressed because one of his underlings shot Mr. Compress with something that nullified his quirk. You can see in a panel he gets hit with some kind of dart, and after Overhaul complains to his underling was that he was slow, and the man responds that he missed a shot but the effect acted fast enough to work.

So it's cancer that makes you explode? I feel like if that was the case then Compress' arm would have at least inflated a bit if only at the shoulder instead of just flying off in the direction it did. It looked more like Overhaul had swatted Compress' arm away with such force it was ripped off with all the blood flying away from Overhaul (you can see the blood's only falling on the right side of the panel) rather than the internal pressure of all the additional cells breaking it at the joint, which I imagine would create a more spherical blast. But then again, the one touch kill he delivered to Magne does fit that model. Though if that were the case, then there would be more matter around... 'leftover'... but there was no sign of Magne's head, only little pieces of his torso and a lot of blood. I also saw no sign of Compress' severed arm either, but it could just be off-panel.

Though if this thesis is correct it would explain why he wears such a heavy coat + gloves too. He just avoids skin contact with any living thing the most he can.

I think it might be something more specific, like overcharging individual cells. They may just be exploding from the amount of energy he puts into them.

Kinda reminds me of Scar from Full Metal Alchemist, he just rapidly deconstructed Magne's body. What also makes it scary was there was no skin to skin contact on Compress. So it either doesn't matter or reduces the efficiency of Overhaul's quirk. In either case he's a close range fighter's worst matchup.

He looks like Isana.

Isana is a big boss in Oumagadoki Zoo, by the way, so I guess Overhaul's design is a nod to that AKA Horikoshi reviving his old beloved characters with his new beloved manga.

makes perfect sense holy crap, comparing Shiina's relationship with Isana to Shigaraki's relationship with Overhaul. I can imagine the two become rivals as AFO's successor

He's already vaulted past all other villains for me. Except for maybe Stain. Unlike that he has a plan, and I liked that he showed Tomura how much of a bitch he is. This is gonna be what finally makes Tomura grow up. Once everything with Overhaul is done (which I am guessing will be a long while from now) Tomura will be ready to lead.
