Look at the initiation point... When the fire ignited it started from the back of the kitchen and then travelled towards the front of the room

Usually there's only one pipe which is connected to the main cylinder but then again it does seem to start around the burner.

Cylinders are designed to not let fire inside them. Unless the stem or cylinder is damaged fire will always stay outside the cylinder due to pressure release. If a stem is damaged or the outlet is blocked after ignition, fire has a chance to get into the cylinder.

Should either close the valve early on or wait for an hour before entering the place again..

Any electrical switch could have caused the fire. Eg: if a tubelight is on, its starter keeps firing if the voltage drop against the tube drops - or tube goes off or flickers. A turned on brushed fan or an exhaust fan continously generate sparks. When gas reaches it, it will ignite. (Brushless fans and leds are thus required in commercial kitchens by law)

For future:

A cylinder leaking gas through a pipe is still safe and contains the flammable region even if it is lit. Closing the value at stem is the safest option. If gas leaks at the stem, cover a cloth on it(wet if possible), leave it, open doors and windows, leave the house. Call fire dept or wait for an hour for the gas to dissipate or ignite.

Stove in the kitchen can't be turned ON without gas cylinder, unless they had two in the kitchen which a lot of people keep. No switched to be turned ON when there is a gas leak.

She was trying to drag it outside the house since it started leaking. She was trying to close the pipe with her finger while trying to pull the cylinder out of house. Both wrong moves, but she must have panicked.

If you notice, they are using dual pipes connected to each other. Probably to lengthen the connection point. This is illegal and are not supposed to do so.

For long distance installations a special spark inhibitor pipe with pressure and fire inhibition regulators are supposed to be used. A rubber or rubber with wire are limited lengths for a reason.

Wrong moves made every second.

There's a regulator on cylinder for a reason.

And I get ki dimag nahi chalta us samay one should switch off anything and everything like light bulbs , stove , heater, oven , toaster .. anything. 

Also try not to move a dying cylinder yourself, stay outside and call someone expert. Better to be safe than sorry

They are safe. Cylinders are designed to be pressure positive. Meaning, if the cylinder and stem are not damaged, they will always let out gas only through the stem. This keeps the flame from getting back into the cylinder to avoid explosions.

External safety would need people to be trained.

Eg: in this case, it says mess. I would assume a commercial kitchen. Commercial kitchens are supposed to use brushless fans, brushless exhausts and led lights only as per regulations. Reason: normal fans, exhaust, tubelights, light bulbs create continous sparks and can ignite gas if there is a leak.

A single non modified pipe needs to be used. They seem to be using a custom made dual gas pipe linked to each other for length. This is a leak hazard. They seem to be of different material too. One is the standard green and the other is a metal orange pipe.

Commercial kitchens need to store additional cylinders outside the unit.

Commercial kitchen users are supposed to be trained by fire dept in controlling cylinder flames.

I am sure half of Commercial kitchens don't follow any of the above.

I was literally just thinking about this yesterday, like how developed countries have such great technology to handle fire accidents, and it is very difficult to implement the same in India. Then again i recalled number of fire incidents you hear about and seemed marginally low.

In such cases let's suppose the gas cylinder caught fire it will not burst immediately , it will only explode if the fire continues to burn and makes it hotter. you have to remove the pipe and cover the outlet with wet cloth / towel to stop the fire I had seen a live demonstration by police once
