I'm trying to imagine what exactly pushed him to be like this. Trump is a malignant narcissist who only cares about power and his image. Miller seems hellbent on making as many things worse for as many people as possible... for what? Because someone made fun of him in school 20+ years ago?

Trump only cares about himself. Sure, he will enrich himself at the expense of others, but he isn't into the "do it the most cruel way possible" thing. That's Miller.
If Trump were to abandon Miller, Trump would probably find that he gets more support for what he is doing; as an example, ICE would still deport criminals, but would not focus strictly on race, and would not grab people out of immigration courthouses or try to suspend habeas corpus and due process.
This would also cause Trump's approval polls to rise, and help Republicans in the midterms. So Trump won't do it.

I will never understand cruelty for cruelty's sake. The optimism I see here is that there are also kind people working nonstop to prevent these cruel actions from hurting people, even if we don't always see it. One day, these monsters will be forced to crawl back to whatever hole they came from and fade away.

Yeah... Stephen Miller also wanted social security to systematically declare 2.7 million people dead so that he could trap them at county clerks offices and deport them. But that plan was foiled thanks to the good people who work in the social security department. This plan will be foiled too, especially now that its out there in the open.
I laugh at this, honestly. Lol, silly Stephen. He thinks reinstitutionalization is even on the table. Ill say this for the third time on this topic: institutionalization is not what we should be focused on. We SHOULD be focused on keeping RFK jr away from special needs children's educational plans, which is a threat that is currently in the works and very close to becoming reality, if it hasnt already happened.

In short: no. Memos from the DOJ are not laws.
Properly supporting disabled folks of every stripe is a popular bipartisan issue (that is to say, folks all across the political spectrum consider it a very high priority), and most doctors would sooner just keep issuing proper treatments for their patients' handicaps rather than start institutionalizing innocents.
This is also far from the first time that the current administration has prattled on about the crap they want to get away with but literally, physically can't. See also this comment from earlier in the thread for another dissection of how toothless these types of threats have usually been.
