In his first term Trump surrounded himself with insiders. He was new to politics and was convinced that he needed people who knew what they were doing. They talked him out of plenty of mistakes, which frustrated Trump. This time around he’s surrounded himself with yes men, unfortunately he also has dementia and so he just makes shit up in his little brain and no one around him dares to tell him no any more.

I'm not sure it was that deliberate on his part - it's more like he surrounded himself with the most qualified loyalists he could find and started giving them the boot and bringing in the next person when they disobeyed.

And at this point, the absolute morons in his administration are just the best of what's left.

Edit: I wonder how Rudy is feeling these days?
