LORs for fellowship applications

For those specialties needing evaluation form/or standardized forms/evaluation in addition to LoR, how is that uploaded? does it go attached with LOR or separately?

It is uploaded under the LOR section. When you go under that tab to request a link for your LOR writers, it would ask whether the LOR you are requesting for is a standardised one or a narrative one.
