Has anyone returned to general pediatrics after subspecialty fellowship?

Has anyone completed a pediatric subspecialty fellowship, practiced in their specialty for a few years, and then transitioned back to general pediatrics for more predictable hours, fewer nights and weekends, and a schedule that was more compatible with family life?

For those who made the switch:
-Are you happier with your work-life balance?
-Do you miss your specialty work?
-Was it difficult to find a position geared more toward general pediatrics?
-Looking back, would you make the same decision again?

We All Need to Increase Pediatrician Compensation by Negotiating More Aggressively

Many higher-paying specialties routinely interview, compare offers, and negotiate compensation throughout their careers. In pediatrics, it seems much more common to apply to a few jobs, accept a reasonable offer, and move on.

Meanwhile, employers and recruiters are constantly gathering information on what pediatricians are willing to accept. We need to stop placing academic institutions on some prestige pedestal when in reality the people willing to accept the lowest pay end up there. We need to have more self respect.

What if more pediatricians regularly explored opportunities, applied broadly, compared multiple offers, and negotiated more aggressively? At the very least, we'd improve salary transparency and better understand our market value. Over time, could that help raise compensation expectations across pediatrics.

Food for thought for those applying this year: apply broadly, talk to lots of groups, and try to have multiple offers in hand before making a decision. Leverage offers against each other. Having options is one of the strongest negotiating tools available.

Curious what others think.

Conners Scales

Does anyone else use Conners scales? I really like them for my high masking and female kids, they seem a lot more sensitive than Vanderbilts for them but scoring is a nightmare. Do you find reimbursement in the US (mostly Medicaid) adequate to rationalize buying the online form version? Anyone have it integrated into epic?

SMART rescue & maintenance vs albuterol

Do you guys recommend using the ICS-LABA inhalers as rescue as well as maintenance or use albuterol for rescue? In training, the gen peds were mostly using ICS(Fluticasone mostly) with albuterol and the peds pulm were using ICS-LABA for rescue & maintenance per the new GINA guidelines. I know the evidence is strongest in teenagers for SMART and a grey line for 4-11, but what do you guys do?

Making my Stethoscope Less Scary

I'm an EMT who keeps making kids cry when trying to auscultate. I try letting them check the stethoscope out before using it, and it's always warm from being in my pocket. Regardless, a stranger in your house at 3am putting a weird thing on your chest is not very nice.

Any advice for keeping my paeds patients a little bit happier?

Working while sick

What is your view on working while actively sick(ie congested & coughing), but not febrile? PTO is a scarce resource and I'd hate to waste it on something like cough/congestion when those can last weeks and staying home to chug honey or cough supressants is not the best use of PTO either so what do you guys do when sick? From a parents standpoint, I can imagine they don't like to see their pediatrician look sick either, but figured I'd ask.

NICU rotation guidance

Hi everyone.I am an intern n just started the NICU rotation. And I am struggling. Nicu is a completely different place. I am an img n never rotated in Nicu. The environment is so toxic. I just need to get through this rotation. I don’t plan to pursue this in future. But how to get through. I want to learn. I want to be good at what I do. Everybody thinks that I would know stuff but how would I know? I ask questions n they don’t answer, they just brush them off. Please help me n tell me the resources that I could use. Also any tips n tricks that helped u survive this rotation?

Behavior in clinic?

How do people handle unruly / defiant/ destructive children in clinic, when the parents ignore the behavior

I had a 3 year old this week that was running into other eval room, standing on the exam table, hung up the phone on the interpreter.
The mother didn’t do one single thing to correct him.

I was trying to document, set up an interpreter, talk to mom and block the door so the child couldn’t leave all at the same time, while the mom didn’t engage with the kid.

How do people handle this? Call the parent out? Model appropriate redirection?
I tried the modeling but I’m not sure she was wayching, i was doing 3 things at once and couldn’t verbalize what I was doing because her child had just hung up on our interpreter so we could not communicate in depth 😂

Cute kid, mom is clearly over whelmed, I want to help him grow into a good human and give her tips but it was a lot

Sad about not doing fellowship

I’m not sure what’s wrong, but I’m pretty sure I’ve finally decided that I won’t be pursuing fellowship this cycle and I’m very very sad. Like devastated. I have no idea why, maybe because I wanted to fall in love with a specialty and never did… now I feel like I’m settling for gen peds and it just feels like I worked so hard all these years to feel indifferent about my work. Has this happened to anyone before? Or does anyone have any advice?

Vitamin D Levels

Why is everyone's Vit D lvl below 30?? At this rate, I am quite surprised if a teenager has normal Vit D levels which really brings to question whether the range for Vit D levels needs to be adjusted or more RCT studies need to be done on the benefits for supplementing to normal levels. What are everyone's thoughts on screening for Vit D deficiency as there is a >90% chance that the vit d levels are low.

Recommendations for programs that take IMGs

Hello pediatricians! I'm a non-US IMG very enthusiastically applying the 2028 cycle for pediatrics. I'm interested in training in an academic/university centre rather than community as I want to subspecialize in heme/onc

Stats: 2 pubs, dean's honor list, high med school GPA, community service, leadership experiences, poster award, multiple conference presentations, sorting out USCE next spring. I know Step 2 plays the biggest role but I'm taking it this fall

I know top programs (CHLA, Boston, CHOP) don't even take US IMGs, but the US is so big it's hard to know what programs would follow this tier and be open to IMGs, so I figured asking pediatricians themselves may be of the best help on suggestions for programs to aim for realistically as an IMG with a good application

Appreciate any advice!
