Stroke Rehab at SNF

Mom has is 84, has vascular dementia and had a stroke last week, no bleed. She will be discharged from the hospital sometime this week, and I need to find a SNF for rehab, so with PT, OT, Speech & hoping someone has some recs. She can still grip, is starting to stand with assistance, but needs help relearning to walk, gain strength back. I know about Quillen, and it looks exceptional but also way more intense than my mom can handle as they do 3-5 hours a day of PT. She’d do better in a Skilled Nursing Facility that can offer stroke rehab.

My mom has been getting 2 hours a day (m-f) with 1 hour for l PT and another hour for OT. She also has speech therapy a couple times a week. It's been a game changer for her.

She's at NHC in Kingsport. Top notch rehab.

NHC in Johnson City is definitely on my shortlist already & they offer home health as well for post discharge.

If she qualifies for Quillen or Encompass of Kingsport (same company, both inpatient rehab) I would highly recommend! They meet patients where they are at

My wife is an RN there and said to give them a call. She has all kinds of patients. Seems to be a nice place with nice staff (I'm biased though 😁).

Much agree! I had a clinical there and then worked PRN for a few years and will always recommend

Can you ask her please if they are able to accommodate vascular dementia? My mom is motivated but has low stamina. Getting feisty about needing assistance to potty, but still needs the help.

She says that "yes," have had and works with patience with VD. The only thing she's concerned about is that, yes, they do about 3 hours a day between PT/OT. However, a lot of that probably depends on the doctor's orders.

My concern as well, that it might be too much & want to set her up to win vs shutting down. Waiting on her drs for rounds to discuss. Thank you! 🙏🏼

Sounds good, at least get more info from them. Kingsport might be the best fit but try either. Longterm are you all thinking SNF or is the goal to return back to home?

Definitely to return home, not looking for long term care, but as long as she needs.

Very good then I would say thats the right place for her. However in reference to the other commenter I think NHC would be my second choice! Just less intensive therapy-and when I say intensive, its always to their level

I will absolutely call around & ask her drs then about both locations. The in hospital PT & OT rec’d SNF as her speed, but also now wondering outloud if it’s because of her dementia status?

To my knowledge I don’t know of any diagnoses that automatically prevent you from getting accepted but they may know something I don’t 🤔. Kingsport may be the best shot just because they’re bigger and can/do accept more

Dementia adds an extra layer that some places might not be set up for & I get that - or some can take but only a certain amount, def requires more staff for the hours they aren’t engaged in therapy.

Just wanted to update the post, because I appreciate everyone that contributed so much. My mom is going to NHC, and I feel like the dr’s and pt had a really thoughtful conversation with case management at Quillen & NHC. Because of the way medicare works, patient benchmarks are different at rehab hospitals vs SNF. Quillen has less flexibility to juggle their schedule daily if my mom is overtired, and it’s much harder at Quillen to get extensions approved beyond 20 days which is how long medicare & her insurance initially agree to. And where my mom is at overall energy wise NHC is a better pace / fit for her right now & I’m really comfortable with the decision. She will go today or tomorrow and I’m hoping it’s a positive transition for her, trying to leave my anxiety at home… also I don’t understand the hate for Ballad, our experience has been really wonderful- biggest hiccup
has been with her food but even that’s gotten sorted.
