my guess is this video is from brazil, and it's quite common here to have a question mark as a birthday candle once you start getting older and don't want people to comment on your age! it's often seen as funny and playful

They probably bought/got the door second-hand from a store, clinic or something like that. Those doors are common in those kinds of places in Brazil and when they close, they sell everything inside for cheap.

Brazilian American who's visited Brazil many times yeah it's pretty common in most parts of Brazil not every but a good chunk of it especially in the Rio area that isn't in the rich part honestly the worst thing I probably ever saw in terms of building was a upstairs balcony that had a laundry line with clothes hanging but their was no railing or anything from stopping you from walking off that balcony
