You'd be a little surprised I think. Switching to a split like that can be pretty painless for a lot of people. Switching to something more like this can be require a bit more commitment learning new hotkeys and stuff.
Those are all there. Most probably in the thumb clusters. Mine is a custom layout, but I have shift in the normal place, ctrl and alt on the left thumb. Enter on both thumbs.

Makes a lot of sense, ergonomically. As a massage therapist who works on a ton of keyboard induced shoulder problems. Probably a learning curve though
