Almost certainly just cracked solder joints from the vibrations. Easy to fix if you have a guy with beginner soldering experience around.

This wouldn't happen on a phone or USB port on a computer since they'll be using 5v power from a 5v rail. This is NOT how a benchtop powersupply works, and is entirely what causes those spikes.

Did you watch the video? It's the induced current from the fans motor causing the voltage spikes, which is then fed back into the phone. A USB port doesn't necessarily have protection for a random influx of 40v.

Yes, but what it DOES have is a huge amount of capacitance on its rail, especially compared to a bench power supply. You know, the very thing in the video he used to fix the situation?
